Only patients receiving treatment at Slim U Canada Inc. may be in the clinic.

If you are experiencing any flu like symptoms however you have a covid negative test 

we respectfully still request you do not come into the clinic for 10 days. 

(We do not want to expose any clients to any flu virus).

Members of the same household are not permitted in the clinic,

we are only permitting one client at a time in the clinic.

All patients must read and acknowledge the

prior to booking a treatment.

Patients violating the terms of the Declaration or any COVID-19 Regulations

may be subject to refusal of treatment.

Patients must always maintain a two (2) meter distance

from Slim U Canada Inc. staff when at the clinic.

changing/moving the laser during phases of the laser therapy session

All patients must fill out a Covid-19 Form Waiver each and every time

they come for treatment.

Patients must declare the following:

Due to the COVID-19 pandemic, we are taking extra precautions with the intake of each client.

Primary symptoms of COVID-19 may include:

* new cough or a chronic cough that is worsening

*fever

*new or worsening shortness of breath or difficulty breathing

* sore throat

* runny nose

Secondary symptoms of COVID-19 may include:

 * stuffy nose

 * painful swallowing

 * headaches

 * chills

 * muscle or joint pain

 * gastrointestinal symptoms

* loss of sense of smell or taste

* conjunctivitis (pink eye)

•I understand the above symptoms and affirm that I, as well as all members of my household, do not currently have nor have experienced COVID-19 symptoms within the last 14 days.

•I affirm that I, as well as all members of my house hold, have not been diagnosed with COVID-19 within the last 14 days.

•I affirm that, to my knowledge, in the last 14 days I have not been in contact with anyone who has been diagnosed with COVID-19

•I affirm that if I traveled outside of Canada in the last month, I isolated in my home for 14 days upon my return.

Cold Laser

PRE-ARRIVAL

Patients must be pre-booked for treatment via phone and email

- no walk-in access.

ARRIVAL & PREPARATION

Patient must wait to enter clinic until 5 minutes or less before their scheduled treatment time.

Only ONE patient is allowed in the clinic at a time.

Patients must be wearing a new mask provided for them prior to entering.

Client must put on a new pair of socks provided for them over their existing socks.

There will be sanitizer available at the front door - Patients must sanitize

their hands prior to entering.

  We will open the entrance door at the exact time of your treatment.

We will be doing a temperature check via forehead non-contact thermometer.

ANYONE with a temperature with more than 37.5 degrees Celsius

will not be allowed to receive treatment.

DURING TREATMENT

Patients must keep their face masks on for the entire duration of their treatment.

POST-TREATMENT

All patients must immediately leave Slim U Canada Inc. upon completion of their treatment.
